We are seeking a dynamic and experienced Care Manager to take on a pivotal leadership role at ValleyView. As Care Manager, you will be responsible for overseeing the care provided to our residents, ensuring it is delivered with consistency, safety, and compassion. You will work closely with the Clinical Manager and Senior Management to maintain and improve the clinical environment, manage the care team, and foster a culture of high-quality service delivery. This role provides a unique opportunity to be at the forefront of care innovation within an aged care setting.Key Responsibilities:

  • Leadership and Team Management:
    Lead the care team by providing mentorship, support, and supervision to ensure that all staff are equipped to deliver high-quality, resident-focused care. Foster a positive and collaborative workplace culture where the wellbeing of both residents and staff is prioritised.
  • Staff Inductions and Onboarding:
    Oversee the induction of new staff members, ensuring that all training materials are current, comprehensive, and engaging. Provide clear, structured onboarding to new employees and agency staff to ensure their successful integration into the team.
  • Care Plan Development and Implementation:
    Collaborate with the Clinical Manager to create, review, and update personalised care plans for each resident, ensuring that the care provided aligns with their individual needs and preferences. Facilitate regular care plan reviews with residents, families, and the clinical team.
  • Feedback and Continuous Improvement:
    Serve as the primary point of contact for receiving and addressing feedback from residents, families, and staff. Work proactively to resolve issues and implement changes that enhance the quality of care and overall resident experience.
  • Emergency Response and Clinical Support:
    Provide direct support during emergency situations, including responding to emergency call bells and overseeing complex care needs. Collaborate with the clinical team to manage resident deterioration, behavioural concerns, and palliative care interventions.
  • Compliance and Quality Assurance:
    Ensure that all care delivery complies with relevant legislative and regulatory standards, including the Aged Care Quality Standards. Work with the Clinical Manager and Senior Management to maintain a culture of compliance and continual quality improvement.
  • Budget and Resource Management:
    Manage care resources effectively, ensuring that all activities are delivered within budgetary constraints. Regularly review departmental spending in consultation with the Business Support Officer and contribute to monthly budgetary reports.
  • Collaboration and Communication:
    Build strong relationships with residents, families, healthcare professionals, and external partners to ensure the holistic care of residents. Attend and contribute to Board meetings, reporting on clinical indicators and addressing any queries or concerns from the Governance Board.
  • On-Call Duties:
    Be available for on-call support during after-hours and weekends, offering guidance to the clinical team and providing emergency assistance when required.Skills & Qualifications:
    • Registered Nurse (RN): You must have a current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
    • Experience: At least 4 years' experience working as an RN in either an aged care or acute care setting. Previous leadership experience overseeing junior clinicians, mentoring staff, or providing training is essential.
    • Clinical Knowledge: Expertise in conducting nursing assessments, administering medications, wound care management, and delivering end-of-life care. Strong understanding of clinical data interpretation, care planning, and evidence-based practice.
    • Leadership and Organisational Skills: Proven ability to lead, inspire, and manage a diverse team of care workers. You will be organised, able to prioritise tasks effectively, and manage competing demands in a dynamic environment.
    • Interpersonal and Communication Skills: Excellent communication and interpersonal skills are essential for building trusting relationships with residents, families, and colleagues. You should be empathetic, culturally aware, and able to engage with people from diverse backgrounds.
    • Compliance and Ethical Practice: Strong understanding of legislative and regulatory requirements governing aged care in Australia. You must be committed to upholding professional standards of practice and adhering to ethical principles in all interactions.What We Offer:
